The ECG shows right bundle branch block and an SI, QIII, TIII pattern. Urgent echocardiogram showed a very dilated right ventricle. Given the high probability of pulmonary thromboembolism, fibrinolysis was performed. At 24hours, computed tomography confirmed bilateral pulmonary thromboemboli and also showed type B aortic dissection (Figure).
